SS-31 at a Glance: A Cardiolipin-Binding Tetrapeptide
SS-31 takes its name from Hazel Szeto and Peter Schiller, the Cornell pharmacologists who designed the Szeto-Schiller family of cell-penetrating peptides. It is a short synthetic chain of four residues, D-Arg-Dmt-Lys-Phe-NH₂, in which Dmt is 2′,6′-dimethyltyrosine. The free peptide has the formula C₃₂H₄₉N₉O₅ and a molecular weight close to 639.8 g/mol. In the literature you will also meet it as elamipretide, its clinical development name, and under the older labels Bendavia and MTP-131. All four names describe one molecule.
Its alternating positive and aromatic residues let it settle at the inner mitochondrial membrane rather than drift in the cell's fluid. Cell studies report concentrations there roughly 100 times higher than in the cytosol. That targeting is why SS-31 is studied differently from general antioxidants: the questions are about membrane structure and energy output, not bulk free-radical clearance.
Elamipretide has a regulatory history of its own. Forzinity (elamipretide HCl) received FDA accelerated approval for Barth syndrome on September 19, 2025. That approval covers a specific medicine in a defined patient group. It does not apply to this vial, which Remy Research supplies as lyophilised material for in-vitro laboratory research. Mechanism: What the Published Research Describes
Most mechanistic data come from cell cultures, isolated mitochondria and animal models. Four themes recur, and each should be read as a research finding rather than an established effect in people.
Two-point grip on cardiolipin
Cardiolipin is a phospholipid found almost only in the inner mitochondrial membrane, where it holds the curved folds called cristae in shape. Birk and colleagues (J Am Soc Nephrol, 2013) used NMR and fluorescence methods to show SS-31 attaching in two ways: its positive D-Arg and Lys residues pair with cardiolipin's negative headgroups, while the Dmt and Phe rings tuck into the lipid tails.
Keeping the electron transport chain organised
The respiratory complexes and ATP synthase sit on a cardiolipin scaffold. In isolated mitochondria put under oxidative stress, SS-31 was reported to preserve complex activity and membrane potential closer to unstressed controls, which researchers link to protection of that scaffold.
Oxidative stress at the membrane surface
Cardiolipin is easily oxidised, and oxidised cardiolipin weakens cristae structure, which in turn can raise reactive oxygen species output. The Dmt residue can scavenge radicals, so SS-31 is studied as an antioxidant placed at the membrane itself rather than in the mitochondrial matrix.
Clinical-development signals, read cautiously
A small 2021 crossover study in older adults (Roshanravan et al., PLOS ONE) reported higher muscle ATP production after one elamipretide treatment. The TAZPOWER trial in Barth syndrome missed both primary endpoints in its blinded crossover phase, then reported a walking-distance improvement in its open-label extension. These are early, small human datasets from a regulated drug programme.
