Tesamorelin at a Glance: A Modified GHRH Peptide

Tesamorelin, coded TH9507 during development, is a man-made version of growth hormone-releasing hormone (GHRH). Its backbone is the full 44-residue sequence of human GHRH. The one change is a trans-3-hexenoyl group attached to the N-terminal tyrosine. Native GHRH is cut quickly in plasma, mainly by the enzyme DPP-IV working at that N-terminal end, and the added group is there to slow that breakdown.

The compound is best known as the active ingredient of Egrifta, a US prescription medicine made by Theratechnologies. The FDA approved it for one narrow purpose: reducing excess abdominal fat in HIV-infected adults with lipodystrophy. The label states that it is not indicated for weight-loss management. That approval belongs to Egrifta alone. The material in this vial is a research reagent, not Egrifta, and it is not an approved medicine anywhere.

Mechanism: Where Tesamorelin Acts on the Growth Hormone Axis

GHRH is the hypothalamic signal that tells the anterior pituitary to release growth hormone. Tesamorelin works at that upstream step rather than being growth hormone itself.

Binding the GHRH receptor

In vitro, tesamorelin binds and activates the human GHRH receptor with a potency similar to the native hormone, according to the Egrifta label. The receptor sits on somatotroph cells in the pituitary.

A stabilised, short-lived molecule

The hexenoyl group improves resistance to enzymatic cleavage, yet the label still reports a mean elimination half-life of roughly 8 minutes in healthy subjects. Its effects are therefore studied through downstream markers rather than circulating drug levels.

Downstream markers: GH and IGF-1

Pituitary release of growth hormone raises IGF-1, and IGF-1 is the marker most trials tracked. In the approved medicine's labelled population, IGF-1 above the normal range was common at 26 weeks, which the label lists as a safety point to monitor.

What did the clinical trials of tesamorelin find?

In two phase 3 trials of the approved medicine in adults with HIV lipodystrophy, a pooled analysis reported visceral fat falling by roughly 15% relative to placebo over 26 weeks, while fat under the skin barely moved. Visceral fat returned after participants stopped. A separate one-year trial in HIV-associated fatty liver disease reported less liver fat. These findings are specific to those populations and to the approved product.

Is tesamorelin approved for weight loss?

No. The Egrifta label says it is not indicated for weight-loss management and describes its effect as weight neutral. Research tesamorelin is not approved for any use.
